Day Habilitation

LifeStream’s Day Habilitation programs provide Medicaid-funded support services for adults with disabilities residing throughout Southeastern Massachusetts. Each of our centers provides individualized medical, therapeutic, and developmental training services to meet the often-challenging needs of people requiring enhanced assistance and support. Our goal is to maintain and enhance personal health as well as foster the greatest possible degree of personal independence. We also offer more specialized programs through our Clinical Services Department.

Referrals to LifeStream’s Day Habilitation programs are generally made by the Massachusetts Department of Developmental Services.
