Adult Family Care

LifeStream is an approved Adult Family Care (AFC) provider in Massachusetts. AFC was developed to provide a residential option for individuals who cannot safely live alone, but do not require the level of support of a more traditional group residence. Importantly, AFC also provides a family setting in the community rather than in a nursing home or other facility.
Recently, the program was expanded to include host caregivers who are caring for relatives in their homes, making them eligible for compensation for providing that care.
Eligibility for LifeStream’s AFC program requires that the individual being cared for is covered by MassHealth Standard or MassHealth CommonHealth and needs assistance coping with challenges from moderate physical or mental disabilities or advanced age. Program participants must be 18 years and older.
